Screening, Brief Intervention, and Referral to Treatment (SBIRT)
We are writing to inform you of a state mandated screening that is new to our school this year. This screening program, as required by Massachusetts law H.4056, is related to the use of alcohol, marijuana, and other substances by middle and high school students. The primary goal of the Screening, Brief Intervention, and Referral to treatment (SBIRT) is to identify and effectively intervene with students who may be at moderate or high risk for problems related to substance use/ abuse. Registration / Data Verification
Formerly the EMERGENCY CARD, the Data Verification includes demographic information, permissions, medical information, over the counter medication permission, and emergency contacts. This information is now collected electronically.
The parent/guardian must complete the Data Verification Form that can be found in the parent portal. This data includes demographic information, permissions, medical information and emergency contacts.
For the safety and well being of the student, this should be completed prior to the first day of school.
