Physicals and Sports Physicals

Physical Exams

All students entering MVRHS must submit a physical examination. Physical examinations must have been performed wothing one year of entrance to school or within 30 days after school entry and at intervals of either three or four years thereafter.

A Student transfering from another school system will be handled as any other new student entering the school.

Health records transferred from the previous school may be used to determine compliance with this request.

Sports Physicals

A sports physical exam must be on file in the nurse’s office before any student will be allowed to participate in any extracurricular sport activity (including tryouts). Sports physicals will be valid for 13 months from the date the physical was performed. If the sports physical expires during the sports season, the student will become ineligible until another sports physical has been submitted to the school nurse. Students may not participate until cleared by the school nurse.

Any type of form may be used as long as it is indicated that the student is “cleared to participate in sports”.

Sports Physicals will be accepted under the following conditions:

1. All sports physical examinations and medical documents must be had delivered, faxed, emailed or mailed to the School Nurse or Athletic Trainer.

DO NOT give physicals, sports physicals, concussion forms, or any other medical documents to the coach.

2. MVRHS follows the Massachusetts Interscholastic Athletic Association’s recommendations:
All students must pass a sports physical examination within thirteen months of the statr of each sports season (Fall, Winter, and Spring).

Sports Physical Exams must be performed by a duly licensed physician (MD or DO), Physician’s Assistant, or Nure Practitioner.

Per State Law 105 CMR 201.000, athletes must also submit a Head Injury and Concussion History Form prior to the start of each sports season.
